Dear Student;

Before you start filling the application form please read the following guidelines carefully:

    (1) All fields in the application form are needed.

    (2) You are allowed to have only one request at a time. You will not be able to make another request until we process your first request.   

    (3) When your request is received you will receive a confirmation message though email address (Please inter a valid email address).

    (4) When your request is approved you will receive a notification message. You must collect the Approval From (Building 14, Room 289).

    (5) Failing to collect the Approval Form within 24 hours (from receiving the approval message) will cancel your request.

    (6) When your request is rejected you will receive a notification message. You can then make another request.
